2008-2009 Application
Admission to the
Laboratory School will require the following:

Dear Parents,
The Southern
University Laboratory School Administration,
Faculty and Staff are pleased to announce that
we are accepting applications for the 2008-2009
school year.
Admission to the
Laboratory School will require the following:

A completed
application form (attached ) with a recent
photograph
-
A cumulative
grade point average of 2.5 or
higher. Student must maintain a 2.5
grade point average for continuous
enrollment at the Laboratory School.

A certified
copy of the most recent report card.
(Conditional acceptance until submission and
review or the final report card).
-
A math and
reading stained score of 5 or higher on any
standardized test taken during the past or
most recent school year (Grades K-3 must
have a DRA grade level score).
-
A copy of
birth certificate (Pre-K students must be 4
years old on or before September 30th
and Kindergarten students must be 5 on or
before September 30th.

A completely
updated copy of student’s immunization
record. (Pre-K students must have all
necessary shots prior to attending class.

A copy of
student’s social security card.
-
Proof of
residency (utility bill, telephone bill,
etc., which shows physical home address).
-
A behavior
statement from the school your child
currently attends and mailed or faxed
directly from that school. The Principal or
Counselor can provide this statement.

A completed
course request form for the 2007-2008 school
year (to be completed for Middle/High school
students upon acceptance of enrollment).

A
non-refundable application processing fee of
twenty-five dollars ($25.00). Money
orders or cashier’s check is the only
acceptable method of payment. No cash
or personal checks will be accepted.

Applicants who wish to be considered for
admission must submit all the required
information. No application will be
reviewed or processes until all required
information has been received. |
Once the
application has been submitted for review, the
Admissions Committee will make final
recommendations to the Director for students who
qualify for admission to the Laboratory School
Fourth grade and
eighth grade applicants must score at the basic
level in ELA and Math on state mandated tests in
order to be considered for enrollment. Tenth
and eleventh graders must have passed ELA, Math,
Science or Social Studies at Approaching Basic
status.
False or
fraudulent statements within the application
will result in denial of admission or immediate
dismissal from the Laboratory School.
